A board-certified family physician and an early adopter of direct primary care, Dr. Chris Ewin founded 121MD in 2003 to make room for a more personal kind of medicine.

Born in Alexandria, Louisiana, Dr. Ewin first became interested in medicine while working summers in his father’s surgical office. He earned his bachelor’s and medical degrees at Tulane University, followed by a pediatric internship at Charity Hospital in New Orleans.

He worked for the Jamaican Ministry of Health in Montego Bay as a District Medical Officer, also supervising Tulane medical students. After a second internship and residency in family medicine at John Peter Smith Hospital, he spent ten years in private practice before founding 121MD.

Beyond the practice

Dr. Ewin has served the Fort Worth community as co-chairman of two capital campaigns for All Saints’ Episcopal School and chairman of the campaign to build the Tarrant County Medical Society building. He is married and has four daughters, three sons-in-law, and seven beloved grandchildren.

Board-certified in family medicine and sports medicine, Dr. Kristin Freeman is passionate about preventive, personalized care and the time it takes to understand the person behind the symptoms.

Born and raised in Arkansas, Dr. Freeman received her Bachelor of Science from the University of Arkansas in Fayetteville and attended the University of Arkansas for Medical Sciences in Little Rock.

She completed her family medicine residency and sports medicine fellowship at John Peter Smith Hospital, where she served as Chief Fellow. She returned to Little Rock to practice non-operative sports medicine in the UAMS orthopedic department and teach residents and medical students as an Assistant Professor.

After returning to Fort Worth to start a family and join a traditional practice, she joined 121MD for the opportunity to give patients individualized care, quality time, and attention to their healthspan and lifespan.

Dr. Kriss Myers is a board-certified family physician with more than 30 years of experience. He joined 121MD in the fall of 2021.

Originally from Longview, Texas, Dr. Myers received his undergraduate degree at Stephen F. Austin State University and earned his medical degree at McGovern Medical School, part of The University of Texas Health Science Center at Houston.

He completed his family medicine internship and residency at John Peter Smith Hospital in Fort Worth. His career in family medicine brings decades of experience to the patients of 121MD.

Dr. Kathe Supavong is a board-certified family medicine and sports medicine physician. Her approach is inspired by the kindness, proven science, and professionalism she saw doctors bring to her own mother’s care.

Born and raised in Dallas, Dr. Supavong developed an interest in medicine as a young person while watching physicians help her mother through multiple medical problems.

She earned her Bachelor of Science at The University of Texas at Austin and her medical degree at Lincoln Memorial University-DeBusk College of Osteopathic Medicine. She completed her family medicine residency at Methodist Charlton Medical Center, serving as Chief Resident, before a sports medicine fellowship at John Peter Smith Hospital.

What isn’t included?

Hospital care, imaging, outside lab work, prescription drugs, specialist services, obstetrics, and emergency care are outside the membership. Review the Medical Services Agreement with the team for full terms.

Membership is not health insurance. Discuss insurance for care outside the practice with your insurance adviser.

Is 121MD right for my family?

121MD offers in-office primary care for adults and children ages six and older. Contact the team to discuss your care needs, physician availability, and whether the membership fits your needs.

How do I become a patient?

Call or email the team. We’ll discuss what you’re looking for, explain membership fees and services, and review the Medical Services Agreement before you join.

How much does it cost?

Contact the practice for current fees. Monthly, quarterly, semi-annual, and annual payment schedules are available. Fees are set out in the Medical Services Agreement.

Does membership replace insurance?

No. Membership covers the agreed primary care services at 121MD. You will need to plan separately for hospital care, specialists, imaging, prescriptions, and other excluded services.

I’m already a patient. How do I get help?

Use the care channel provided by your physician for appointments, refills, and clinical questions. If you need access instructions or scheduling help, call 817.423.5121. Please do not send medical details to the general email address.

What if I need emergency care?

For a medical emergency, call 911 or go to the nearest emergency department. The general practice email is not an emergency service.
